Objectives: Febrile neutropenia (FN) creates concern in paediatrics due to the risk of serious bacterial infections (SBI). Protocols with empiric antibiotics designed for hematology and oncology are often applied in healthy children with FN despite lower rates of SBI in this population. This study quantifies rates of infections in presumed immunocompetent children hospitalized with suspected viral illnesses and FN. Methods: This was a retrospective chart review of healthy children admitted to the Stollery Children's Hospital between 2007 and 2017 with fever, absolute neutrophil counts < 0.5 × 109/L, and viral symptoms. Primary outcomes were the incidence of SBI and bacterial pneumonia. Results: Of 383 encounters reviewed, 96 admissions for 82 patients met inclusion criteria. Eighty-eight encounters (91.7%) were managed with empiric antibiotics. Viruses were identified in 42% of encounters. Three blood cultures were positive for coagulase-negative Staphylococcus and one for Coryneforms, all considered contaminants. There were three urinary tract infections and two pneumonias. Eighty-three per cent of patients had normalization of neutrophil counts, with a median neutropenia duration of 3.2 months. Follow-up diagnoses included chronic benign neutropenia of childhood (N = 17) and three rheumatologic/autoimmune conditions (N = 3). Conclusion: Our results support previous findings of low rates of invasive bacterial infections in healthy children with FN. With an SBI rate of 3.1% and few patients found to have any pathologic etiology for their neutropenia, prospective studies would be valuable to evaluate the need for a practice change regarding antibiotic use in low-risk patients with suspected viral-induced neutropenia.

Resumo Servidores públicos comprometidos e orientados por valores do éthos público é um dos principais desafios da gestão de pessoas. Conhecer o que lhes dá sentido ao trabalho pode ser um caminho para ações pertinentes, fora de estigmas e más interpretações sobre a relação entre o indivíduo e seu trabalho no setor público. O propósito deste artigo é avaliar os fatores antecedentes da experiência de sentido do trabalho, bem como sua relação com bem-estar psicológico, sofrimento psicológico e comprometimento afetivo em peritos criminais da Polícia Federal. Trata-se de pesquisa de natureza quantitativa, com uso de análise fatorial confirmatória e modelagem de equações estruturais. Os resultados indicam que os peritos veem sentido em seu trabalho, o qual advém de sua utilidade social e das oportunidades de aprendizagem e desenvolvimento. O sentido está positivamente relacionado com bem-estar psicológico e comprometimento afetivo e negativamente com sofrimento psicológico. São discutidos os resultados da aplicação do modelo à luz de um marco de responsabilidade gerencial para desenvolvimento da função pública e conclui-se que ele oferece boa contribuição para gestão de pessoas no setor público possibilitando discutir fatores que favorecem a experiência de comprometimento e qualidade de vida psíquica no ambiente de trabalho.


Resumen Servidores públicos comprometidos y guiados por los valores del ethos público es uno de los principales retos de la gestión de personas. Saber lo que les da sentido al trabajo puede ser una forma para acciones pertinentes, fuera de los estigmas y conceptos erróneos acerca de la relación entre el individuo y su trabajo en el sector público. Este estudio tiene como objetivo evaluar los factores de fondo de la experiencia de sentido en el trabajo y su relación con el bienestar psicológico, la angustia psicológica y el compromiso emocional a los expertos forenses de la Policía Federal. Se trata de una investigación cuantitativa, utilizando el análisis factorial confirmatoria y modelos de ecuaciones estructurales. Utilidad social del trabajo y de aprendizaje y desarrollo de oportunidades eran la mayoría de los factores de fondo importantes. El significado del trabajo se relaciona negativamente con la angustia psicológica y positivamente con el compromiso afectivo y bienestar psicológico. Los resultados de la aplicación del modelo se discuten a la luz de un marco de responsabilidad de gestión para el desarrollo de la función pública. Se llegó a la conclusión de que ofrece una buena contribución a la gestión de personas en el sector público que permite analizar los factores que favorecen la experiencia del compromiso y de la calidad de la vida psíquica en el lugar de trabajo.


Abstract One of the main challenges of human resources management in public administration is to make sure that civil servants are committed to and driven by values of the public ethos. Understanding the elements that give meaning to their work can be a way to take pertinent action, leaving aside the stigma and misinterpretation of the relationship between individuals and their work in the public sector. This article aims to assess the antecedent factors of our central construct, meaning of work, and its relationships with psychological well-being, psychological distress and affective commitment for forensic experts in the Federal Police. It is a quantitative study that uses confirmatory factor analysis and structural equation modeling. The results show that forensic experts find meaning in their work through its social utility and the opportunities they have to learn and develop. Meaning is positively related to psychological well-being and affective commitment, and negatively related to psychological distress. This study discusses the results of applying this model in light of a managerial accountability framework to develop the civil service and concludes that it offers a good contribution to human resources management in the public sector, making it possible to discuss factors that favor the experience of commitment and psychological quality of life in the work environment.

RESUMO

The purpose of this study is to develop and validate the Basic Psychological Needs at Work Scale (BPNWS) in French, but items are also provided in English in the article. The BPNWS is a work-related self-report instrument designed to measure the degree to which the needs for autonomy, competence, and relatedness, as identified by Self-Determination Theory (Deci & Ryan, 2000), are satisfied at work. Using exploratory and confirmatory factor analysis, the first study examines the structure of the BPNWS in a group of 271 workers. The second study tests the measurement invariance of the scale in a group of 851 teachers from two different cultures, Canada and France. Results support the three-factor structure and show adequate internal consistency, as well as nomological validity across samples.

Recent research has demonstrated that the perception of injustice at work may increase psychological health-related problems. The purpose of this study is to examine the moderating effect of coworker support and work autonomy on the relationships between both distributive and procedural justice and psychological distress. Results, on the basis of responses to questionnaires given to 248 prison employees, show that coworker support moderates the relationships between both forms of justice and psychological distress. Specifically, these relationships are weakened when employees benefit from a high level of coworker support. Furthermore, work autonomy moderates the relationship between procedural justice and psychological distress but not the relationship between distributive justice and psychological distress. Thus, procedural injustice is less likely to increase psychological distress when the level of work autonomy is high.
